Forms for New Members:

• Release of Liability Waiver (ADULT) - required before attending any class.

• Release of Liability Waiver (MINOR) - must be completed and signed by parent and/or guardian for all participants 17 years of age and under.

• New Membership Form - required if purchasing any membership, class pack or introductory class cards.  Not needed for drop-ins or single classes.
